  • The APC Annual Meeting is accredited for a maximum of 26.0 hours.  Claim only those hours for which you were in attendance; pay careful attention to which concurrent session you attended.
  • Please do not skip any questions; however "comments" are optional. Partially completed applications cannot be processed.
  • Please evaluate each session as to how successfully it met the stated educational objectives of the course.  If you did not attend a particular session, select "0" hours attended and “Did not attend” as the evaluation in the second column.
  • The deadline for submitting your CME Application is September 15, 2010. Your application will be processed, and CME certificates will be sent by October 15, 2010.
